O endere\u00e7o \u00e9 na Rua Jacutinga \u2013 antiga travessa 7, n\u00ba 1.600 (estacionamento da Igreja da Congrega\u00e7\u00e3o Crist\u00e3 do Brasil). <\/span><\/span><\/span><\/p>\n<p align=\"justify\"><span style=\"color: #171717;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">Na sexta-feira (19), das 8h \u00e0s 13h, com o \u201cM\u00e9dico na Pra\u00e7a\u201d, na Vila Hort\u00eancia, Zona Leste da cidade. O endere\u00e7o \u00e9 na Rua Manoel Lopes, 441, Vila Hort\u00eancia.<\/span><\/span><\/span><\/p>\n<p align=\"justify\"><span style=\"color: #171717;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">A a\u00e7\u00e3o conta com uma cl\u00ednica m\u00f3vel e estrutura com acomoda\u00e7\u00e3o para aguardar a consulta, permitindo o acolhimento por demanda espont\u00e2nea de adultos. \u00c9 necess\u00e1rio apresentar documento de identifica\u00e7\u00e3o com foto. O atendimento visa trazer diagn\u00f3stico precoce de condi\u00e7\u00f5es cr\u00f4nicas, como hipertens\u00e3o e diabetes, al\u00e9m de fortalecer o v\u00ednculo entre a popula\u00e7\u00e3o e a rede de sa\u00fade.<\/span><\/span><\/span><\/p>\n<p align=\"justify\"><span style=\"color: #171717;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">A iniciativa atua como extens\u00e3o da rede assistencial, sendo direcionada prioritariamente a bairros e territ\u00f3rios com maior vulnerabilidade social ou que apresentem barreiras geogr\u00e1ficas que dificultem o acesso aos servi\u00e7os de sa\u00fade. <\/span><\/span><\/span><\/p>\n<p align=\"justify\"><span style=\"color: #171717;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">Tamb\u00e9m s\u00e3o ofertados aferi\u00e7\u00e3o de sinais vitais (press\u00e3o arterial, glicemia capilar, entre outros), atendimento de enfermagem, al\u00e9m da consulta m\u00e9dica do projeto.
